Low-dose digital X-rays if they're needed — we'll skip them if we can use recent images from another office.
Findings, options & next steps
We explain what we found in plain language and, if treatment is recommended, give you written options — no pressure to decide on the spot.
What to bring
A few things help us make the most of your time — bring what applies to you.
- Photo ID — a driver's license or other government-issued ID.
- Dental insurance information, if you have coverage — your card or plan details so we can verify benefits.
- A list of current medications and any major medical history.
- Relevant dental records or recent X-rays, if you've been seen elsewhere in the last 1–2 years.
- A heads-up about dental anxiety — if past visits have been difficult, tell us when you book and we'll plan accordingly.
When you book, we'll email a secure new-patient intake form to complete before you arrive, so check-in is quick. Please don't send health details through the website — bring them to your visit or use the secure form.

Yes. Dr. Matthew Phillips is welcoming new patients of all ages at 3rd Set Smiles in Tempe, AZ. You can book online through Zocdoc or call the office at (480) 334-2752.
Your first visit is unhurried. You'll meet Dr. Phillips and the team, talk through your concerns and goals, review your relevant health and dental history, and have a comprehensive exam. Digital X-rays are taken when appropriate, and we review what we find and your options together — with a written summary if treatment is recommended and no pressure to commit.
Bring a photo ID, your dental insurance information if you have coverage, a list of any current medications, and any recent dental X-rays or records if you've been seen elsewhere in the last year or two.

New patients without insurance can start with our $49 new-patient special (comprehensive exam and full-mouth digital X-rays); a standard cleaning can be added for $50, or $99 for exam, X-rays and cleaning. Deep periodontal cleanings and treatment-related X-rays aren't included, and the offer can't be combined with insurance benefits.
